Ulefnersen
Also known as ION363, jacifusen
Bottom line
One placebo-controlled Phase 3 trial met its main goal, according to the companies' topline press release; full data are not yet presented or peer reviewed. An earlier open-label series of 12 people found most still declined, though two did unusually well.
Ulefnersen is an antisense medicine (a short piece of genetic code) given by spinal injection for people whose ALS is caused by a change in the FUS gene. In September 2026 the companies announced that its Phase 3 FUSION trial met its main goal, combining function and survival, compared with placebo. The full results have not yet been published or peer reviewed. It is not approved yet; Otsuka and Ionis are discussing a fast-track filing with the FDA and other regulators.

Evidence: One positive trial
Studies: 1 for · 1 mixed
One controlled trial met its main goal, but no second trial has confirmed it yet.
FUSIONMet its main goal
Met its main goal (function, rescue treatment and survival combined) at 72 weeks in the 73 people in the main analysis (p=0.0005), and NfL improved. Topline press release only; ALSFRS-R point differences and full data not yet published.
Jacifusen expanded access case seriesMixed
Investigator-led, no placebo group. The nerve-damage marker NfL fell sharply, but most people kept losing function and two died. One young woman regained walking and breathing, and one person without symptoms stayed symptom-free.

CompanyIonis Pharmaceuticals (licensed to Otsuka)Explanation and sources
Ionis, a US company that specialises in antisense (RNA-targeted) medicines, discovered and developed ulefnersen and ran the FUSION trial. In November 2024 Otsuka, a large Japanese drugmaker, took exclusive worldwide rights; Otsuka now leads regulatory filings, manufacturing and sales, while Ionis receives milestones and royalties.

How it worksLowers the faulty FUS protein that damages motor neuronsAntisense oligonucleotide against FUS pre-mRNAExplanation and sources
In FUS-ALS, a changed FUS gene makes a harmful form of the FUS protein that builds up in motor neurons. Ulefnersen sticks to the FUS gene's messenger RNA so the cell makes less FUS protein, including the harmful form. It is injected into the spinal fluid so it reaches the brain and spinal cord directly.

StagePhase 3Phase 3 positive (company topline only); FDA talks under wayExplanation and sources
FUSION, a placebo-controlled Phase 1-3 trial in 89 people, reported positive topline results on 22 September 2026. The companies say they will review the results with the FDA and other regulators to look for a faster (expedited) route to approval. No approval application had been announced as of 5 November 2026.

Best reported effectMet main goal in Phase 3 (topline, not yet published)Joint rank of survival, rescue treatment and ALSFRS-R at 72 weeksExplanation and sources
The main goal combined three things: time to death or permanent breathing support, time to needing rescue treatment, and change in the ALSFRS-R function score over about 72 weeks. In the 73 people in the main analysis, ulefnersen did significantly better than placebo (p=0.0005), and blood neurofilament (a nerve-damage marker) also improved. These are company topline results; the companies have not yet said how many points of ALSFRS-R were saved, and the full data have not been presented or peer reviewed. Earlier, in a 12-person open-label case series, most people still declined, though two did unusually well.

Who it's forOnly people with ALS caused by a FUS gene mutationFUS · Spinal injection (intrathecal)Explanation and sources
FUS-ALS is rare (about 0.6% of people with ALS) but often starts young and progresses fast. You need a genetic test showing a FUS mutation to be eligible. The drug is not being studied in other forms of ALS.

What's nextFDA discussions on an expedited filing; full FUSION data at a congress and in a journalExplanation and sources
Otsuka and Ionis said they will move quickly to review FUSION with the FDA and other regulators, and will present detailed results at a medical meeting and submit them to a journal "in the coming months". They have not given a date for an approval application.

Can I join?Early access programme via treating doctors; trial no longer enrollingExplanation and sources
FUSION has finished enrolling. In September 2026 Otsuka opened a global early access programme for people with a confirmed FUS-ALS genetic diagnosis who cannot join a trial. Requests must come from the treating doctor, not from patients or families, and each case still needs regulatory, ethics and treatment-centre approvals, so timing varies.

Signal: Active
Signal measures how often, and how reliably, Ionis Pharmaceuticals (licensed to Otsuka) tells the public about this drug. It says nothing about whether the drug works. A quiet company isn't necessarily a bad sign, but regular, on-time updates help patients plan.

FUSION met its main goal: ulefnersen beat placebo on a combined measure of function and survival (p=0.0005), with supporting results on blood NfL. Full data will come at a medical meeting and in a journal.
Otsuka announces FUSION results and plans to talk to the FDA
Otsuka, which holds the rights, said it will review the results with the FDA and other regulators to look for an expedited path to approval.
Otsuka opens a global early access programme
People with a confirmed FUS-ALS genetic diagnosis who cannot join a trial may get ulefnersen through an early access programme. Requests must come from the treating doctor.
Reuters reports ulefnersen met its main Phase 3 goal
Independent coverage noted there is no approved treatment aimed at the genetic cause of FUS-ALS and that the companies will pursue accelerated approval routes.
Ionis says FUSION readout is due in the second half of 2026
On its second-quarter call, Ionis said it expected late-stage results from several partner programmes in the second half of the year, including ulefnersen for FUS-ALS with Otsuka.
FUSION reaches its primary completion date
According to the trial registry, the last participant finished the main (double-blind) part of FUSION around this date. The trial enrolled 89 people and is no longer recruiting.
Ionis lists FUSION results among its 2026 readouts
In its outlook for 2026, Ionis named results from the FUSION study of ulefnersen (partnered with Otsuka) as one of the Phase 3 readouts expected that year.
2025
Lancet case series on 12 people treated under expanded access
Columbia-led doctors reported 12 people with FUS mutations treated with jacifusen (the earlier name of ulefnersen). The nerve-damage marker NfL fell by up to 83%; most people still declined, but one recovered function and another stayed symptom-free.
2024
Otsuka licenses ulefnersen from Ionis
Otsuka took exclusive worldwide rights to ulefnersen for FUS-ALS, paying US$10 million up front. Ionis keeps running the Phase 3 FUSION trial, with Otsuka covering costs from 2025.
