Masitinib
Also known as AB1010, Masitinib AB Science
Bottom line
One trial reported slower decline in a selected group of patients, but not in everyone treated. European and Canadian regulators judged the data unreliable and refused approval; the confirmatory trial has not started.
Masitinib is a daily pill meant to calm overactive immune cells (mast cells and microglia) that may feed inflammation in ALS. An earlier trial suggested slower decline in a selected group of patients, but Europe's medicines agency refused approval twice (2018 and 2024) and Health Canada also turned it down in 2024, citing unreliable data and no benefit in the full study population. A confirmatory Phase 3 trial was authorised in 2025 but never started, and the company now hopes to begin it in late 2027 after fixing problems found by regulators.

Controlled trials missed their main goal. Some measures or subgroups looked better, which needs confirming.
~27% slower decline in selected group; data disputed
3.4 points less decline over 48 weeks on the 48-point ALSFRS-R (normal progressors only; no difference in all patients)
AB10015 re-analysis (patients before any complete loss of function)Mixed
A company analysis of a narrower group chosen after the trial reported longer survival. It is a preprint, not peer reviewed, and Health Canada considered this group post hoc.
AB23005 (confirmatory Phase 3)Pending
About 408 people planned over 48 weeks. Authorised in 2025 but no patient enrolled; suspended in Europe, with a restart aimed for late 2027.
AB10015Mixed
Met its main goal in "normal progressors" (3.4 points less decline at 48 weeks), a group set by a mid-trial amendment; no clear benefit in all patients. Regulators found study-conduct problems.

CompanyAB ScienceExplanation and sources
AB Science is a small Paris-listed company. Masitinib is its lead drug and has been tested in many diseases; in 2026 the company dropped several of those programmes to focus on ALS and one cancer drug. It has relied on repeated small share sales and debt deferrals for funding, and it changed chief executive in July 2026. In September 2026 inspectors from three European regulators carried out a "for cause" inspection of how it runs clinical trials.

How it worksCalms overactive immune cells that may fuel nerve inflammationTyrosine kinase inhibitor (mast cells, microglia; c-Kit, LYN, FYN)Explanation and sources
Masitinib blocks signalling enzymes (kinases) in immune cells called mast cells and microglia. In ALS these cells may become overactive and keep up inflammation that harms motor neurons. The idea is that calming them slows nerve damage. In 2026 the company reported a blood marker linked to inflamed microglia that it wants to test in the Phase 3 trial, but has not named it.

StagePhase 3Phase 3 authorised but never started; suspended, restart aimed Q4 2027Explanation and sources
The confirmatory Phase 3 trial AB23005 (about 408 people, 48 weeks) was cleared by the FDA and several European countries in 2025, but no patient has been enrolled. In April 2026 the company paused all European trials after regulators raised questions about its resources, and EU regulators listed AB23005 as suspended. After a regulatory inspection in September 2026, the company said it now aims to restart the trial in the fourth quarter of 2027, subject to health authority approval.

Best reported effect~27% slower decline in selected group; data disputed3.4 points less decline over 48 weeks on the 48-point ALSFRS-R (normal progressors only; no difference in all patients)Explanation and sources
In the earlier AB10015 trial, people progressing at a "normal" rate (losing under 1.1 points a month) lost 9.2 ALSFRS-R points on high-dose masitinib plus riluzole versus 12.6 on placebo plus riluzole over 48 weeks. This group was defined by a mid-trial change to the plan, and there was no clear benefit in the whole group treated or on survival. Serious side effects were more common on masitinib (31% vs 18%). European regulators found problems with how the study was run and did not consider the data reliable. Later company analyses of narrower subgroups are post hoc and partly unreviewed preprints.

Who it's forSlower-progressing ALS without complete loss of any functionEarly stage · Pill, daily (with riluzole)Explanation and sources
The Phase 3 trial is designed for people with ALS whose decline is under 1.1 ALSFRS-R points a month and who still score at least 1 on every item of the ALSFRS-R (no function completely lost). The company says this is about three-quarters of people with ALS. US participants may also take edaravone.

What's nextRestart of the Phase 3 AB23005 trial (subject to regulators)Expected Q4 2027Explanation and sources
AB Science said on 29 September 2026 that it must first finish a new quality management system and have it audited by the end of 2026, then pass a new inspection, before resuming the ALS trial in the fourth quarter of 2027. The final inspection report is expected before the end of 2026.

Signal: Active
Signal measures how often, and how reliably, AB Science tells the public about this drug. It says nothing about whether the drug works. A quiet company isn't necessarily a bad sign, but regular, on-time updates help patients plan.

2026
Half-year 2026 results confirm revised ALS timeline
The company repeated that the ALS Phase 3 trial should resume in the fourth quarter of 2027, with the final inspection report due by year end.
Regulatory inspection pushes ALS trial to Q4 2027
After a "for cause" inspection by French, Danish and Greek regulators, AB Science said it must rebuild its quality systems first and now aims to resume the ALS Phase 3 trial in late 2027.
Three other masitinib trials dropped; ALS protocol to be revised
AB Science stopped trials in mastocytosis, mast cell activation syndrome and multiple sclerosis, and said it would amend the ALS Phase 3 protocol before seeking permission to start.
Survival data to be presented at ENCALS 2026
The company presented its long-term survival analysis of the earlier trial at the European ALS meeting in Madrid.
Full-year 2025 results focus resources on ALS
AB Science said it would concentrate on masitinib in ALS and one leukaemia drug, and strengthen its organisation before launching the ALS trial.
EU registry lists the ALS Phase 3 trial as suspended
Regulators temporarily suspended AB23005 as a precaution while issues with the sponsor's trial conduct were investigated. The trial had not started.
Company preprint on long-term survivors
AB Science reported that 42.3% of people on masitinib in the earlier trial were alive five years after first symptoms, compared with historical figures rather than placebo. It is a preprint.
European trials paused; insurance offer for ALS Phase 3
AB Science paused recruitment in its European trials after regulators questioned its resources, and announced an insurance policy that would repay Phase 3 costs if the trial fails.
Company reports a blood biomarker linked to masitinib's target cells
AB Science said it found an unnamed blood marker of inflamed microglia that it plans to add to the Phase 3 trial. The findings come from in-house experiments.
2025
Company preprint on a narrower subgroup of the earlier trial
AB Science posted a look-back analysis of people with no complete loss of any function, reporting larger benefits. It is a preprint and has not been peer reviewed.
ALS News Today reports Phase 3 masitinib trial can launch
Coverage explained the 408-person trial design and that US patients on edaravone can take part.
Confirmatory Phase 3 trial authorised in first European countries
Spain, Greece and Slovenia cleared the AB23005 trial, following earlier FDA clearance. It targets slower-progressing patients without complete loss of any function.
2024
EMA confirms refusal of masitinib for ALS
After re-examination, Europe's medicines agency kept its negative opinion. It found no benefit across the whole study population and judged the trial data unreliable because of problems with how the study was run.
