Stopped
DNL343
Bottom line
The drug reached its target in an early study, but a large placebo-controlled trial found it did not slow ALS or lower a nerve-damage marker. Denali has stopped its ALS work.
DNL343 was a pill designed to switch off a stress response inside cells that is linked to clumps of the TDP-43 protein, found in most people with ALS. In the HEALEY ALS Platform Trial it did not slow ALS or change a key nerve-damage marker, and Denali stopped its ALS work in 2025.

Evidence: Not shown to work
Studies: 1 against
Controlled trials found no benefit.
No benefit vs placebo
About the same decline as placebo, 0.87 vs 0.84 points a month on the 48-point ALSFRS-R
HEALEY ALS Platform Trial, Regimen GNo benefit
Missed its main goal: decline in function and survival over 24 weeks was about the same as on placebo. Muscle strength, breathing and the nerve-damage marker NfL did not differ either.

CompanyDenali TherapeuticsExplanation and sources
Denali Therapeutics is a US biotech company developing medicines for neurodegenerative diseases. It provided DNL343 to the HEALEY ALS Platform Trial as Regimen G and later wound down the programme.

How it worksCalms a cell stress response linked to TDP-43 clumpseIF2B activator (integrated stress response inhibitor)Explanation and sources
When cells are stressed, they switch on the integrated stress response, which slows protein production and can lead to clumps called stress granules. In ALS these are linked to TDP-43, a protein that clumps in motor neurons. DNL343 activates eIF2B, a protein that helps turn this stress response off, in the hope of protecting neurons.

StageDiscontinuedFailed in HEALEY Platform Trial (Phase 2/3); ALS work ended 2025Explanation and sources
Denali announced in January 2025 that DNL343 missed its main and secondary goals. In March 2025 it reported no effect on neurofilament light, a blood marker of nerve damage, and stopped the extension study. HEALEY researchers later said further study of DNL343 in ALS is not warranted.

What happenedMissed main and secondary goals in HEALEY trial; no effect on NfL; ALS work ended6 Jan 2025Explanation and sources
The January 2025 topline results showed no benefit, and a follow-up analysis showed no effect on nerve-damage markers. Denali stopped the extension and HEALEY investigators concluded further ALS study was not warranted.

Best reported effectNo benefit vs placeboAbout the same decline as placebo, 0.87 vs 0.84 points a month on the 48-point ALSFRS-RExplanation and sources
In the trial, 186 people received DNL343 and 139 received placebo for 24 weeks. The published results showed a disease rate ratio of 1.04, where 1.0 means no difference, so decline was about the same as placebo. It also did not lower neurofilament light.
