CNM-Au8
Also known as gold nanocrystals
Bottom line
Both placebo-controlled trials missed their main goals. It lowered a blood marker of nerve damage (NfL), and company analyses link this to longer survival, but no trial has yet shown that people live longer or function better.
CNM-Au8 is a drink containing tiny gold crystals designed to help nerve cells make and use energy. Its two Phase 2 ALS trials missed their main goals, but it lowered a blood marker of nerve damage (neurofilament light, NfL) and company analyses link that drop to longer survival. In October 2026 Clene asked the FDA for accelerated approval based on this marker; the FDA has not yet said whether it will review the application.

Evidence: Hints only
Studies: 3 mixed
Controlled trials missed their main goal. Some measures or subgroups looked better, which needs confirming.
Missed main goals in 2 trials; NfL marker ~10% lower
No difference in function: both groups lost about 1.0 ALSFRS-R point a month over 24 weeks (HEALEY)
Company NfL and survival analyses (HEALEY, RESCUE-ALS, expanded access)Mixed
Clene's later analyses link NfL lowering to longer survival, partly using comparison groups from outside the trials. They underpin the FDA application but cannot by themselves show that the drug helps people live longer.
HEALEY ALS Platform Trial, Regimen CMixed
Missed its main goal (function and survival combined) and all three secondary goals over 24 weeks. The NfL nerve-damage marker was lower than on placebo, and there were fewer deaths on the 30 mg dose, both exploratory findings.
RESCUE-ALSMixed
Missed its main goal (a nerve-signal measure called MUNIX) and its breathing goal over 36 weeks. In later follow-up fewer people first given CNM-Au8 had died (7 vs 15), an exploratory finding in a small trial.

CompanyClene Nanomedicine (Clene Inc.)Explanation and sources
Clene is a small company based in Salt Lake City, Utah, and CNM-Au8 is its only drug, also studied in multiple sclerosis. Money is tight: it had $9.9 million in cash at the end of September 2026, which it says lasts only until the middle of the first quarter of 2027. More money from warrants depends on the FDA accepting its application for review.

How it worksGold nanocrystals that help nerve cells make energyCatalytic gold nanocrystals (NAD+/NADH, mitochondrial energy)Explanation and sources
The gold crystals are meant to speed up chemical reactions that cells use to produce energy, helping stressed motor neurons and their support cells survive and work better, and reducing harmful oxidative stress. This mechanism comes mostly from lab studies and is not proven in people.

StageFiled for approvalFDA accelerated approval application submitted Oct 2026Explanation and sources
Clene submitted a New Drug Application in October 2026 asking for accelerated approval, a pathway where the FDA can approve a drug based on a marker (here NfL) that is "reasonably likely" to predict benefit, with a later trial needed to confirm it. The FDA first decides whether to accept the file for review, then whether to approve. A confirmatory Phase 3 trial, RESTORE-ALS, has not started.

Best reported effectMissed main goals in 2 trials; NfL marker ~10% lowerNo difference in function: both groups lost about 1.0 ALSFRS-R point a month over 24 weeks (HEALEY)Explanation and sources
In the HEALEY ALS Platform Trial, CNM-Au8 did not slow decline on the ALS functional score compared with placebo, and the earlier RESCUE-ALS trial also missed its main goal. In HEALEY, NfL, a blood marker of nerve damage, rose about 8% on placebo and fell about 2% on CNM-Au8 over 24 weeks, an exploratory finding. Company analyses, many done after the fact or using comparison groups from outside the trial, link NfL drops to longer survival. It is not yet proven that lowering NfL with this drug helps people live longer or function better.

HEALEY and the expanded access programmes included adults with sporadic or familial ALS. Clene's application covers ALS generally rather than one genetic form; the exact proposed label has not been published.

What's nextFDA decision on whether to accept the NDA for reviewExplanation and sources
After an application is submitted, the FDA first decides whether it is complete enough to review. Clene has not given a date for this decision. Separately, it plans to start the RESTORE-ALS Phase 3 trial in the second quarter of 2027.

Can I join?Expanded access at some US ALS centres; Phase 3 not yet startedExplanation and sources
ClinicalTrials.gov lists one CNM-Au8 expanded access programme (NCT05281484) as available at participating US sites; ask your ALS clinic whether it takes part. The NIH-funded programme (NCT06408727) is listed as temporarily not available. No placebo-controlled trial is enrolling; RESTORE-ALS is planned for 2027.

Signal measures how often, and how reliably, Clene Nanomedicine (Clene Inc.) tells the public about this drug. It says nothing about whether the drug works. A quiet company isn't necessarily a bad sign, but regular, on-time updates help patients plan.

FDA meeting minutes support filing an NDA based on NfL
The FDA said Clene's data "may be capable of supporting" an accelerated approval application using NfL, and asked for evidence that the size of the NfL drop predicts benefit. Clene targeted Q3 2026.
Clene raises up to $28 million tied to FDA milestones
Part of the money depends on the FDA accepting the application for review and part on approval, via warrants that investors can exercise later.
2025
Expanded access data show lower NfL and GFAP with CNM-Au8
Clene reported lower NfL at week 36 in the NIH-funded expanded access programme and lower NfL in the HEALEY trial. The expanded access results are not compared against placebo.

2024
FDA gives Clene a possible route to accelerated approval
After earlier saying the data were not enough, the FDA suggested Clene gather more NfL blood-marker data from its expanded access programmes. Clene then aimed to file in mid-2025 and start Phase 3 first.
